The cardiac rehabilitation program at the Wellness Institute offers a comprehensive, individualized program in an environment that encourages lifestyle changes that last a lifetime.
Through exercises, assessments, dietary counseling, and more, our highly trained staff will help you understand and reduce risk factors associated with the progression of cardiovascular disease.
What to Expect
To tailor this program to your needs, you will undergo an evaluation and meet with a registered nurse and an exercise physiologist at the beginning of the program.
Individualized exercise programs and care plans are developed to support lifestyle changes needed to reduce risk factors and allow you to better manage your health future.
The program includes a total of 36 sessions, which take place over 12 weeks. Sessions are offered on Monday, Wednesday, and Friday at two different times:
- 7:45 a.m. – 10:00 a.m.
- 9:15 a.m. – 11:30 a.m.
Sessions include exercise, education, diet and nutrition counseling, and psychosocial support. To get the most out of the program, regular attendance is very important.
Certification
Durham Regional Hospital’s cardiac rehabilitation program is licensed by the North Carolina Department of Facility Services and nationally certified by the American Association of Cardiovascular and Pulmonary Rehabilitation.
Insurance
Cardiac rehabilitation is a covered benefit by Medicare, Medicaid, and most commercial insurances. Wellness Institute staff will verify benefits and obtain pre-certification as needed.
Medicare patients must begin rehab within 12 months of cardiac event; private payers require attendance within six months.
Participation Requirements
A physician referral is required to take part in cardiac rehabilitation. You may qualify for the program if you have experienced:
- Heart attack
- Angina
- PTCA, stent
- Valve repair or replacement
- CABG
- Heart transplant
Refer a Patient
Physician referrals are required for enrollment in Wellness Institute programs. To refer a patient, fax the cardiac rehabilitation referral form (DOC, 28 KB) to 919-470-8156. Please be sure to clearly state the patient’s name, diagnosis, and contact information.
After receiving a referral form, we will follow up with the patient to obtain insurance information and coordinate enrollment in the appropriate program.
