Setting Goals

The discharge planning process begins on the day of admission. The plan sets goals for the patient and addresses the expected post-hospital needs of the patient. By having a discharge plan with clear, specific goals, the patient is able to measure progress and work to improve and refine abilities so he or she may enjoy an improved quality of life.

At Durham Rehabilitation Institute, the patients spend very little time in their rooms. They are actively involved in therapy, leisure activities, and gaining new skills and abilities that will allow them to maximize their potential.

DRI offers the combined talents of both Duke and community medical specialists at one convenient location with easy access and free parking.

Each patient’s treatment team is led by both the patient’s personal physician and the program manager. Other team members may include:
