Registration is located on the second floor of the hospital. Elevators to the second floor are at the rear of the main lobby and stairs are located behind the information desk.
Registration is open from 6 am-6 pm Monday through Friday.
We strongly recommend that you pre-register prior to the day of your procedure. To pre-register, simply come to registration during registration hours. If you are unable to pre-register, please allow yourself an extra hour for registration the day of your exam.
Bring your photo identification, insurance card and referral or doctor’s order with you. Applicable payment of co-insurance or deductibles will be required at this time.
